Our website offers a choice of five different order forms depending on the type of order you are placing.
They all use our current list of titles and prices and provide extensions and an order total,
which allow you to create an accurate list of needed materials.
To find the correct form, you may use the list with descriptions immediately below or
view a list of users and the order forms they should
use. In either list, click on the underlined order
type for detailed instructions.
Choose an Order Form that matches your situation:

School District Order Attachment
For use by a Florida Public School District or school therein to create a list to be attached to
a valid District Purchase Order and delivered to Florida School Book Depository.
Adopted and non-adopted
titles may be ordered along with qualified free materials, as allowed by your district's policy.
The order should be printed at your location and attached to a valid District Purchase Order before
being delivered to Florida School Book Depository; it cannot be submitted directly to the vendor
through this website.

School District Requisition Attachment
For use by a Florida Public School District or school therein to create a substitute Florida State
DOE District Requisition. This form should be used to order Florida State
adopted materials and
associated free materials only. It should not be used to purchase non-adopted
material with flex
funds. The order should be printed at your location and attached to a valid District Purchase Order
before being delivered to Florida School Book Depository; it cannot be submitted directly to the vendor
through this website.
Current Account Order - NOT for use by public schools using a district purchase order
For use by the following:
- A Florida Public School using internal or other non-district funds to pay for the order by issuing
a school check.
- A Florida Private School or Charter School with a credit-approved open account in good standing.
- A Florida educational institution or organization with a credit-approved open account in good standing.

This order may be used in lieu of a Phone Order, Fax
Order, or mailed Purchase Order if in accordance
with the procedures of your institution. The order created may be submitted directly to Florida School
Book Depository via e-mail from this website or delivered by other means (but do not send duplicates).
This form cannot be used for orders using Public School District funding or by parties that do not have
a credit-approved open account in good standing.

For use by Florida State Certified Teachers, State Licensed Schools, and Customers without credit-approved
open accounts in good standing. This order cannot be submitted directly from this website and must be
accompanied by remittance including transportation.

For use by Florida Home Schools, non-certified teachers and tutors, parents, and other Florida residents.

District Purchase Order. Or in purchasing systems requiring line item entry of each title ordered,
the form can be used as an accurate source document. The list created does not constitute an order;
a valid District Purchase Order must be provided and, therefore, it cannot be submitted directly
through this website.

The School Prices shown in our catalogs and database are available to all Florida licensed schools,
educational institutions and certified teachers. They are updated regularly, but do not constitute a
quote. The School Price for State Adopted titles is the same as the State contract price. State contract
prices can be escalated at the beginning of the fourth year of the State contract by an amount established
by the DOE. In the case of other materials, the School Price is the Publisher's net wholesale price, which
we offer as their agent, and is subject to change without notice. The price charged on all invoices is the
price in effect at the close of business operations the day the order is processed. All prices are F.O.B.

Many free teaching materials are available upon request with your order of qualifying student materials.

next business day making extra overnight charges unnecessary. If you would like to pick up your order,

by the Florida Department of Education.
Intended Use
To create a substitute Florida State DOE District Requisition Form. This form should be used for Florida
State Adopted materials and associated free materials only. It should not be used to purchase non-adopted
material with flex funds. The form should be printed at your location and attached to a valid District
Purchase Order prior to being delivered to Florida School Book Depository; it cannot be submitted directly
through this website.
Identifying State Adopted Titles
Adopted titles are shown in our catalogs in ALL CAPITAL LETTERS; non-adopted titles are in
upper and lower case.
Adopted titles in the database and on the order forms are indicated by the display of the adoption expiration year after the
title description; non-adopted titles do not display a date. Titles with no year date following the title
should only be ordered on the Requisition Attachment if free, unless your district allows otherwise.

- To create an order that can be used as or attached to a School Purchase Order.
- To submit an order that otherwise would be called in by phone. Using this form will provide clear
documentation of your order and give you fast, easy service.
- To submit an order that otherwise would be sent by fax. Using this form will save you the trouble
of busy fax lines and, possibly, long distance call charges.
Orders created may be e-mailed from the website upon completion or printed and sent by other means.
Be careful not to send duplicates of the same order by the same or different means. The customer is
responsible for duplicated orders if we receive more than one copy.
Do not use this form for Public School District orders because they require that a purchase order be
attached.
Do not use this form if you are a Private School or organization that does not have a credit-approved
open account in good standing.
