Apply for Credit

Thank you for your inquiry regarding the establishment of a line of credit with FSBD.

FSBD is a private company serving the Florida public school system, private schools, charter schools, libraries and other tax-exempt educational institutions.

We require a completed Credit Application, and a copy of your Tax-Exempt Certificate to begin the credit approval process.

By signing the FSBD Credit Application, you agree to our terms of net within 30 days.

The following credit information forms are available in Adobe PDF format. The PDF files require that you have Adobe Acrobat Reader which is available for free.

Once the credit investigation is complete, you will be contacted regarding your account number and line of credit. If you should have any questions, please contact our Accounting Department at accounting@fsbd.com or call 1-800-447-7957.
